I, the undersigned, acknowledge that I am voluntarily participating in fitness, yoga, meditation,
movement, and/or wellness-related sessions offered by Zaazen. I understand and agree to the
following:
Assumption of Risk
I acknowledge that activities including (but not limited to) yoga, aerial yoga, movement
classes, physiotherapy, recovery sessions, and use of wellness equipment (e.g., cold plunge, red
light therapy) carry inherent risks of physical injury. I voluntarily assume full responsibility
for any risks, injuries, or damages that may arise from my participation.
Medical Conditions
I affirm that I am in good physical condition and do not suffer from any condition that would
prevent or limit my participation. I agree to inform Zaazen of any relevant medical or health
conditions prior to participation.
Consultation with Healthcare Provider
I acknowledge that Zaazen instructors and practitioners are not licensed medical professionals
and do not diagnose, treat, or prescribe. It is my responsibility to consult a physician before
beginning any new wellness or fitness program.
Release of Liability
In consideration of being permitted to participate, I hereby release, waive, and discharge
Zaazen, its owners, instructors, therapists, employees, and contractors from any and all
liability, claims, demands, or causes of action related to any loss, injury, damage or death
resulting from my participation.
Photography & Video Consent
I consent to the use of any photographs or video recordings taken during classes or events for
promotional purposes on social media, website, or print material. (please inform us if you do
not wish to be photographed)
Class Booking & Attendance Policy
I understand that all classes must be pre-booked, cancellations must be made as per our
cancellation policy hours and missed sessions may count as a no-show as per Zaazen's policy.
I have read and understood this waiver and release. I voluntarily agree to its terms and understand
that by signing this document, I am waiving certain legal rights.
